Does anyone know how to add a hidden date function to this that will attach the date of submittion of the form? 
I am not sure why there would be a date format $date_fmt if you couldn't do this.

But it does indicate the date on the submitted form. The script already includes it, you do not need a hidden tag.

Do you not get the following on your emailed results?

I too facing the same problem in my mail servers, i don't how it could be happen. Anyone else to solve this.
