Help with Spam Assassin and Email

I'll preface by saying I'm a 3D animator/motion graphic designer and not any kind of web guru.

We've been getting a lot more obvious spam as of late, and as far as I can tell Spam Assassin isn't catching any of it.  I see on other posts that Spam Assassin has had a fair number of problems on LP.

We have our default account which is our LP user name.  Within that we have three individual accounts tied to our domain name.  When I accessed our default account yesterday via Horde I saw folders for our three individual accounts, plus folders called SPAM, spam, Junk, Trash, a few test accounts and Virtual Folders. 

Spam Assassin is ENABLED. 

Spam auto-delete is DISABLED.

Spam box is currently ENABLED.

With these settings, though, the "spam" folder had nothing in it newer than Feburary 2012.

Telling Spam Assassin to clear the spam boxes DOES remove whatever messages are in the spam folders (including recent emails I manually moved to the spam folder).

With this framework in mind, a few questions:

How does Spam Assassin know what folders to send spam to?  Does the folder have to be named "spam" or "SPAM" or "Spam" or what?  Some of the individual accounts did not appear to have "spam" folders, so I added those.

Does each individual account have to have one of these explicitly-named spam folders, or do I just need one spam folder in our default account?

How do I even know whether Spam Assassin is doing anything?  I disabled and enabled Spam Assassin.  A window with some code popped up:

I have no idea what any of the code means.  After re-enabling no obvious spam (i.e.: Your Starch Inhibitor) is being diverted into any of the spam folders.  I sent some emails from another account with "VIAGRA" in the title, and they weren't caught.

Do I have to do something in cPanel regarding Account Level Filtering or User Lever Filtering to make Spam Assassin work?

Sorry for all of the newbie questions.  I created a support ticket yesterday regarding this, and the reply that came 24 hours later was useless; the responder obviously didn't bother to read my ticket.

After further research I created a spam email filter using Account Level Filtering in cPanel based on these instructions:

That said, I don't see any evidence that Spam Assassin is doing anything.  A filter test with "Viagra pills" in the title returns these results:

Return-path copied from sender
Sender      = xxxx@xxxx.lunarservers.com
Recipient   = xxxx@xxxx.lunarservers.com
Testing Exim filter file "/etc/vfilters/xxxxx.net"

Filtering did not set up a significant delivery.
Normal delivery will occur.

Hi WShawn, spam assasin should work. There might be something that has been missed here. Or those viagra emails might just got smarter. Have you created a support ticket yet and have this one investigated by an LP staff?

If SA was running, I think you should have gotten something in the header telling you the SA score. So, I would suspect that SA isn't even running. I've read a lot of complaints about SA crashing and LP doesn't bother to monitor it, acting only when someone opens a support ticket. A ticket is probably your next step.

Spam asassin does not work on LP. Not a single email was classified as SPAM for me in a long long time.

There are two issues with SA on LP. One is when it quits, in which case you don't see any X-Spam-Status or X-Spam-Score headings in your emails. That can be fixed with a support ticket.

The other problem is when SA is running, but doesn't score correctly due to outdated rules. In that case, a lot of spam has scores so low it comes through like ham. Unfortunately, no one has found a fix for this problem yet, and LP hasn't even commented (that I know of -- if anyone has a reference please correct me).

In the meantime, I've found Mailwasher (http://www.mailwasher.net/mailwasher-free) to be an invaluable tool in quickly spotting and deleting spam. It should be safe to mention, since it's not a competing product. It's one of the few programs that has a good freeware version that I've paid for the pro version of.

I can't comment on your particular server, but I can say that spamassassin is definitely working properly on mine (more than one).
