I just uploaded PhpBB3.  However, when I go to my website: www.declutteratyourservice.com - I get the following:

Can someone assist me?

As it appears you are on a Cpanel based server, the things you will want to check are:

1. Try enabling php5 on the account via the "php configuration" icon in cpanel, as the newest version of phpBB I believe requires php5.

2. Make sure that you have put in the correct database information into your script.

Hostname: localhost
Username: cpanelusername_mysqlusername
Password: Password set for mysql username
Database name: Cpanelusername_databasename

3. If the issue continues, contact support at support@lunarpages.com (email) or support.lunarpages.com (browser) to check it out.

Be sure to include:

Domain name having issue:
Account primary domain name or username:
Last 4 of card on record (security purposes, needed in case changes are needed and can save time including it up front)
Exact database configuration information used.
Location of database connection file:
