Your account would just be upgraded. It's a simple procedure.

The maximum space is on the premium plan, you get 600Mb.

You can buy additional space for $2 per 25 megs per month.

What happens when someone buys a 12-month standard plan, and then has to upgrade to Premium?  Do you just charge the difference for the remaining months on the original contract?  Or maybe start a new 12 months period?

If you decide to upgrade, you will be pro-rated (pay the diffrence) only. There are no extra costs.
