LP can meet all those requirements (there is an extra fee for SSH, but there are other ways to do things without SSH). If this is for personal or business use, the best rate I know of is $4.95/month for 24 months, paid in advance (use coupon 'spring'). Maybe if you look around, you can find a discount or coupon for a better introductory rate. If this is for a school, LP used to have a free K-12 program -- I don't know if they still do. If after all this you can't fit within your budget of $40/year, you'll probably have to go with a free hosting service (but they tend to be quite restrictive in what you can do, and limited in resources).
