On the phpbb forum, I try to ban someone and get this error

File : admin_user_ban.php




I checked the database and it seems to be working OK.  Also I have never modified the admin_user_ban.php file at all.   I've looked at the phpbb forums and there is no help there.  I need to ban a user and have no idea why I can't.  

Can anyone help me?

Does this error happen for all users or just one user?

If just one, then the SQL error means that the entry in the database already exists.

It happens to all users.  I've been to the database and it does show the one that I have entered before, about a month ago.  Also I was oddly able to get an IP address in there when it was giving me the errors.  I kept trying and it took once. Now it doesn't seem to work at all.  I have upgraded my board but this file has never changed.  Unless the database did.  Not sure if it did with the upgrade.

It seems like that table isn't autoincrementing.

i would backup the database, then try removing and re-adding that table..
