I would like to have a 3 question survey pop-up on my website's homepage asking my users some questions. I would like to be able to save the answers in some format - either a basic db or a csv file, etc. I installed PHPSurveyor but is way to robust for my needs - and it requires an email invitation for users to access the survey where I am trying to get people as they visit my site.

Any ideas for what type of script that I should look for? Anyone out there done something similar with their site and willing to bestow some help?
