Is this something for a shopping cart (e-store) or a forum? Have you looked at the support and discussion sites for your shopping cart or forum to see if such a thing is available as an add-on? For example, osCommerce has a product review system that might do exactly what you want. If not, I'm sure someone has written a "five-star" add-on that would plug right in. The next step would be to look in more general script sites such as Hotscripts.com and NotOneBit.com. The only problem with these is that they may not integrate well with a shopping cart or forum, but would work well in a more stand-alone (custom coded page) situation.
