There's tons available at the CGI Resource.  The most popular through the years have been Matt's scripts, but they are considered insecure nowadays and you would be encouraged to use the nms replacements.

So on that page - to email a form to yourself you would probably want to use nms mail, although I find that TFMail is easier to configure.

If you search for NMS Mail or TFMail on these boards you will find threads outlining installation and setup issues.  There are also threads where you can get help on any problems you encounter in setting up the scripts.
