I'm thinking I need to take an instructor-led course to learn how to create a database-driven website using PHP and MySQL with Dreamweaver.  Anyone know of a good online school that offers this course (a la carte)?

Dreamweaver isn't going to be the least bit useful for working with PHP and MySQL. At best, you can use it to WYSIWYG build a static HTML framework of what the page will look like, and then salvage the generated HTML and CSS for use in the PHP. If all you're using it for is a code editor, I would suggest that you learn a real flat file editor such as ViM or Notepad++. In the long run you'll be much happier and more productive, and you'll have a much larger choice of schools to choose from.

I have to agree  with MrPhil. The best way to lear MySQL is to learn the basics about SQL. I am doing the same thing on a one month course with www.Lynda.com  It will cost you $25 for the month.   
If you are having problems installing the MySQL server and configuring it to work with Dreamweaver get in touch. It took me ages to find out how to do it successfully.

Try Googling "using dreamweaver with php and mysql". There are plenty of tutorials out there from sites like Adobe, Sitepointe and even Princeton University.
