This is the code I have in my form and I keep getting an error. Can someone provide  the correct code?

You can either check the wiki or knowledge base as you seem to be using a simple formmail script which is not accepted on the servers due to they are easily exploited. You may want to use PHPlist or some other mailing list. Also the server security scan picks up on anything names mail, contact, and so on. The reason this is so due to the fact that spammers scan sites looking for such words in effort to see if it can be exploited for spamming purposes.
